Insect bites
Skin reaction to bites — mosquitoes, bees, spiders, ticks and others. Most are benign, but some situations deserve assessment by a dermatologist.
What are reactions to stings
Insect bites and other arthropods (mosquitos, bees, wasps, spiders, ticks, fleas, bedbugs) cause local and, in some cases, systemic reactions. Most of them are light, self-limited and simple care.
Some situations — extensive reactions, signs of infection, systemic allergy, tick bites — deserve medical evaluation. An assessment by a dermatologist helps distinguish bites from other conditions that resemble them.
Common reactions
- Reddish papule, with itching or burning at the site of the bite.
- Longer reactions — redness and swelling several centimeters around the bite; they can last days.
- Multiple in-line or group stings (suggestive flea, bedbug).
- In bee stings/vespa — severe immediate pain, with local swelling.
- In spider bites — small papules with two bite points; some species cause local necrosis (rare in Portugal).
- In tick bites — persistent papule with the insect sometimes still attached; specific risk of disease transmission.
Alarm signs — seek urgency
In the following situations, you should seek immediate medical evaluation — online consultation does not replace urgency:
- Difficulty breathing, wheezing, hoarseness or "closed throat" sensation.
- welling of the tongue or throat
- Dizziness, pallor, loss of consciousness after bite.
- Generalised reaction with extensive urticaria.
- Bee sting/vespa on someone with known allergy.
- Signs of rapidly evolving local infection — extended redness, fever, severe pain.
Diagnosis
The diagnosis is clinical. In an online consultation, the dermatologist evaluates photographs of the lesions and a context questionnaire (open air activity, travel, pets), evolution time and symptoms.
Care
Local care
Wash the area with soft water and soap. Local cold reduces itching and swelling. Do not scratch — reduces the risk of secondary infection. Elevation of the area in the bites on the limbs helps reduce swelling.
Topical or systemic treatment
In more intense reactions, the dermatologist may indicate specific formulations to control itch and inflammation.
Safe tick removal
Remove as soon as possible with tweezers, next to the skin, with firm traction and without twisting. Watch the area for the next few weeks — if erythema appears in a ring, seek medical evaluation.

Frequently asked questions
How to distinguish mosquito bite from bedbug bite?
Bedbugs often bite online or in groups of 3 ("breakfast, lunch, dinner"), typically in areas exposed during sleep (neck, arms). Mosquitoes are more dispersed. The context (travel, accommodation) helps distinguish.
When is the tick bite worrying?
Watch the area for the next four weeks. If erythema appears in a ring (migratory erythema), fever, muscle or joint pain — seek medical evaluation. Could be a sign of Lyme disease.
Is online consultation for stings?
Yeah, for cases with no signs of systemic alarm. The dermatologist evaluates by photo and guides care. Severe or systemic reactions require hospital urgency.
Do I need a tetanus shot after a bite?
Not usually, but if you have the outdated vaccination schedule and there is an associated trauma wound, it may be recommended. Check with your family doctor.
